SCHOOL SUPPLY PACKAGE ORDER FORM

Pick up date: August 27, 2003 from 9 A.M. – 3 P.M. in the OMS main lobby

Attention all 6th and 7th Graders:
This year we are offering you the opportunity to order needed school supplies for the middle school. Listed on the attached page are the items you can purchase by preordering now. We will be accepting orders until June 19th. Complete the order form and return with payment (check payable to: Oceanside Middle School) to your classroom teacher. The supplies listed in Packages A & B are required for all students on all teams. Other supplies will also be available at our school store throughout the year.

Orders must be picked up on August 27th between 9:00 a.m. and 3:00 p.m. in the main lobby of the middle school. If you are unable to pick up your orders, they can be picked up during extended advisory at the school store on the first day of classes.
